1. 基本方針
「通院・入院準備ノート」（以下「本アプリ」）は、利用者が通院・入院準備に関する情報を端末内で整理するためのアプリです。アカウント登録を必要とせず、本アプリの運営者（以下「当方」）が運営するサーバーへ利用者の記録を自動送信しません。
3. 権限と端末機能の利用
- 通知：利用者が設定した受診・入院予定を端末内で通知するために使用します。
- Face ID、Touch ID、端末のパスコード：利用者がアプリロックを有効にした場合の本人認証に使用します。当方および本アプリは生体認証情報そのものを取得しません。
- 連絡先選択：利用者が緊急連絡先として明示的に選択した氏名と電話番号を端末内へコピーするために使用します。連絡先全体の継続的な同期は行いません。
- ファイル・写真：利用者が選択した資料の保存、バックアップの保存・復元、PDF・CSVの保存または共有に使用します。
4. 外部送信、共有および第三者提供
本アプリには広告、行動追跡、利用状況分析、当方のクラウド同期機能はありません。当方は本アプリから個人情報または健康関連情報を受領せず、販売または第三者提供しません。
利用者がPDF、CSVまたはZIPバックアップを保存、印刷、共有した場合、選択した保存先・共有先へ情報が渡ります。この操作は利用者の明示的な指示によりiOSのシステム画面を通して行われます。共有先サービスにおける取扱いは、その提供者のプライバシーポリシーをご確認ください。
5. 保存期間と削除
情報は、利用者が本アプリ内で削除するか、本アプリを端末から削除するまで端末内に残ります。外部へ保存したPDF、CSVまたはZIPバックアップは、本アプリ内の削除やアンインストールでは削除されません。利用者自身で保存先から削除してください。
本アプリはアカウントや当方のサーバーを使用しないため、当方が遠隔で端末内データを閲覧または削除することはできません。
6. セキュリティ
本アプリは、任意でFace ID、Touch IDまたは端末のパスコードによるアプリロックを利用できます。アプリロックは画面へのアクセスを制限する機能であり、端末内データ、写真、PDF、CSVおよびZIPバックアップを独自に暗号化する機能ではありません。共有先と保存先を利用者自身でご確認ください。
7. 医療上の注意
本アプリは、利用者が入力した情報を整理するためのツールです。診断、治療、服薬の判断、健康状態の監視または緊急医療を提供しません。医療上の判断については、医師、薬剤師その他の医療従事者にご相談ください。
8. 子どもの利用
本アプリは子どもを対象として設計されたサービスではありません。保護者が家族の記録を管理する場合は、適用される法令と本人のプライバシーに配慮してご利用ください。
9. 本Webサイトについて
当方は本Webサイトに広告、アクセス解析、問い合わせフォームまたは行動追跡機能を設置していません。言語切り替えはURLの表示設定だけに使用し、Cookieには保存しません。Webサイトの配信事業者が、セキュリティと配信のためにIPアドレスや標準的なアクセスログを処理する場合は、その事業者の規約が適用されます。
10. 本ポリシーの変更
機能、法令またはデータの取扱いが変わる場合、本ポリシーを更新します。重要な変更がある場合は、本アプリまたは本ページ上で案内します。
1. Overview
Visit & Admission Notes (the “App”) helps users organize information for clinic visits and hospital admissions on their device. No account is required, and user records are not automatically sent to a server operated by the App provider (“we,” “us,” or “our”).
3. Permissions and device features
- Notifications: Used for local visit and admission reminders chosen by the user.
- Face ID, Touch ID or device passcode: Used when the user enables the optional app lock. We and the App do not receive biometric templates.
- Contact picker: Copies only the name and phone number explicitly selected by the user for an emergency contact. The App does not continuously access or synchronize the address book.
- Files and photos: Used for user-selected attachments, backup and restore, and user-initiated PDF, CSV or ZIP save and share operations.
4. Transmission, sharing and third parties
The App has no advertising, behavioral tracking, developer-operated usage analytics or cloud synchronization. We do not receive personal or health-related information from the App, and we do not sell or disclose it to third parties.
When a user chooses to save, print or share a PDF, CSV or ZIP backup, information goes to the destination selected by the user through an iOS system interface. The destination provider’s privacy policy applies to its handling of that information.
5. Retention and deletion
Information remains on the device until the user deletes it in the App or removes the App. A PDF, CSV or ZIP backup saved outside the App is not deleted when an in-app record is deleted or the App is removed. The user must delete those files from the chosen destination.
Because the App does not use accounts or our servers, we cannot remotely view or delete information stored on the user’s device.
6. Security
The App offers an optional app lock using Face ID, Touch ID or the device passcode. It limits access to the screen; it does not independently encrypt on-device records, photos, PDFs, CSV files or ZIP backups. Users should review the security and recipients of any destination they choose.
7. Medical notice
The App organizes information entered by the user. It does not diagnose conditions, recommend treatment or medication, monitor health conditions, or provide emergency services. Users should consult qualified healthcare professionals for medical decisions.
8. Children
The App is not designed as a service directed to children. A parent or guardian who manages a family member’s records should follow applicable law and respect that person’s privacy.
9. This website
We do not place advertising, analytics, contact forms or behavioral tracking on this website. The language setting is stored only in the page URL and not in a cookie. A website hosting provider may process IP addresses and standard access logs for security and delivery under its own terms.
10. Changes to this policy
We may update this policy when the App’s features, applicable law or data practices change. Material changes will be communicated in the App or on this page.
