1. 適用と同意
本利用規約は「通院・入院準備ノート」（以下「本アプリ」）の利用条件を定めます。本アプリをダウンロードまたは使用することで、本規約およびAppleの標準エンドユーザ使用許諾契約に同意したものとします。適用される強行法規またはAppleの条件が本規約と異なる場合、それらが優先します。
2. 本アプリの目的
本アプリは、利用者が自ら入力または選択した通院、入院準備、薬、費用、写真資料などの情報を端末内で整理するためのツールです。医療機関のシステム、電子カルテ、行政サービスまたは保険サービスとは連携しません。
3. 医療上の注意
本アプリは医療機器ではなく、診断、治療、処方、服薬の可否・用量判断、健康状態の監視、緊急医療または医療上の助言を提供しません。本アプリの表示や準備率のみを医療判断に使用しないでください。体調、治療、薬または緊急時の対応については、医師、薬剤師、地域の救急サービスなど適切な専門機関へご相談ください。
4. データの管理とバックアップ
入力内容は利用者の端末内に保存されます。利用者は、入力内容の正確性、端末の管理、必要なバックアップ、書き出したPDF・CSV・ZIPの保存先と共有先について責任を負います。当方は端末内データを遠隔で閲覧、復元または削除できません。機種変更、端末故障、アプリ削除の前にZIPバックアップを作成してください。
アプリロックは画面へのアクセスを制限する機能であり、端末内データや書き出したファイルを独自に暗号化する機能ではありません。
5. 利用上の責任と禁止事項
利用者は、適用法令と第三者のプライバシーを守って本アプリを使用してください。本人の同意なく家族その他の第三者の情報を不適切に記録・共有すること、本アプリの妨害、解析、権利侵害または違法な目的での使用を禁止します。ただし、適用法令が認める範囲を不当に制限するものではありません。
6. 購入、返金およびAppleのサービス
本アプリはApp Storeを通じて提供されます。価格、支払い、領収書、返金、ファミリー共有、再ダウンロードおよび地域別の提供条件はAppleの規約とApp Storeの表示に従います。本アプリ内にサブスクリプションまたは追加課金はありません。購入や返金に関する手続きはAppleのサポートをご利用ください。
7. 知的財産権
本アプリ、Webサイト、名称、文章、画像、デザインおよびプログラムに関する権利は、当方または正当な権利者に帰属します。本規約は、本アプリを個人的に利用するための限定的な使用許諾を与えるもので、権利を譲渡するものではありません。利用者が入力した内容の権利は利用者または各権利者に留保されます。
8. 提供、更新および変更
当方は、安全性、互換性、法令、機能改善その他の理由により、本アプリまたは本規約を更新、変更または提供終了する場合があります。すべての端末、OSまたは将来の環境での継続利用を保証するものではありません。重要な変更は本アプリまたは本Webサイトで案内します。
9. 保証と責任の範囲
本アプリは現状有姿で提供されます。当方は、入力内容の正確性、医療上の結果、データの完全な保存、特定目的への適合性または中断のない動作を保証しません。適用法令で認められる範囲で、本アプリの利用または利用不能、データの消失、外部共有先での取扱いから生じる損害について責任を負いません。ただし、消費者保護法その他の強行法規により制限できない権利や責任は除きます。
1. Acceptance and scope
These Terms govern the use of Visit & Admission Notes (the “App”). By downloading or using the App, you agree to these Terms and Apple’s Standard End User License Agreement. Mandatory applicable law and Apple’s terms control if they conflict with these Terms.
2. Purpose of the App
The App is a tool for organizing information that users enter or select about visits, admission preparation, medications, expenses and photos on their device. It does not connect to provider systems, electronic health records, government services or insurance services.
3. Medical notice
The App is not a medical device and does not provide diagnosis, treatment, prescriptions, medication decisions, health monitoring, emergency care or medical advice. Do not use a displayed checklist or preparation percentage as the sole basis for a medical decision. Consult qualified healthcare professionals or local emergency services about health, treatment, medication or emergencies.
4. Data and backups
Content is stored on the user’s device. The user is responsible for the accuracy of entered information, device security, necessary backups, and the recipients and destinations of exported PDFs, CSV files and ZIP backups. We cannot remotely view, recover or delete on-device data. Create a ZIP backup before changing devices, removing the App or when device failure is a concern.
The optional app lock limits screen access. It does not independently encrypt on-device data or exported files.
5. Responsible use
Use the App in accordance with applicable law and the privacy rights of others. Do not improperly record or share another person’s information without appropriate authority, interfere with the App, infringe rights, or use the App for an unlawful purpose. This does not restrict rights that cannot lawfully be limited.
6. Purchase, refunds and Apple services
The App is distributed through the App Store. Price, payment, receipts, refunds, Family Sharing, redownloads and regional availability are governed by Apple’s terms and the App Store listing. The App contains no subscriptions or additional in-app purchases. Use Apple Support for purchase or refund requests.
7. Intellectual property
Rights in the App, website, name, text, images, design and software belong to us or their respective rights holders. These Terms provide a limited license for personal use and do not transfer ownership. Rights in content entered by users remain with the user or the applicable rights holder.
8. Availability, updates and changes
We may update, change or discontinue the App or these Terms for security, compatibility, legal, product or other reasons. Continued availability on every device, operating system or future environment is not guaranteed. Material changes will be communicated in the App or on this website.
9. Disclaimer and limitation of liability
The App is provided “as is.” We do not guarantee the accuracy of user-entered content, medical outcomes, complete preservation of data, fitness for a particular purpose, or uninterrupted operation. To the extent permitted by law, we are not liable for loss arising from use or inability to use the App, data loss, or handling by an external destination selected by the user. Rights and liabilities that cannot be limited under consumer or other mandatory law remain unaffected.
